Running 800 MeV/A d through He-4 gas (ca 1 m), I have approximately once per 2e6 events reports as below.

Using: Geant4 version Name: geant4-09-06-patch-01    (1-February-2013)

(On a debian squeeze machine, geant4 locally compiled)

It does not crash, only reports.  Was not able to find this in the bugzilla.

Though I don't think 1 in 2 Million is frequent-;
