When a neutron is captured on hydrogen, two deuterons instead of one deuteron and
a 2.2 MeV gamma are produced (see attachment).

I am using Geant4.9.5 with the QGSP_BERT_HP physics list (G4NEUTRONHPDATA=G4NDL4.0). 

The error was did not occur with Geant4.9.4.p01 (G4NEUTRONHPDATA=G4NDL3.14).

Thank you very much for reporting this problem.

I believe the fix of this problem was committed but unfortunately it does not include in the release.
So it will be included next patch release.
 You still have this problem after the patch, then please let us know.
