Since 4.9.4, this example no longer works. Building and running with the defaults, using run.mac, should write out dose to the file dicom.out. Indeed, it works with 4.9.3.p02. May be related to bug 1209.

There is a bug int the DICOM example: the name of the SD starts with a capital in one place and not in the other. Could you check if it happens the same in your case? Oh, sorry. I am posting comments to Problem 1209, but Pedro, you are commenting here. I would close this bug, because with the SD name correction, the DICOM example then works out-of-the-box, so to speak, since voxel boundaries are skipped by default. Problem 1209 is not specific to the DICOM example: dose is not scored in any G4PhantomParameterisation when using SetSkipEqualMaterials(false).
